Patient Navigator Program
Familias en Acción helps individuals with and without health insurance, who have been diagnosed with any of the following conditions: diabetes, hypertension, kidney failure, cancer, HIV, substance dependence, any chronic disease, and others. Patient navigators help patients and their families through education about cancer and other chronic conditions, to understand their condition and know the different treatment options that are accessible for them. Patient navigators can assist patients and their families to find resources such as accessing financial help, housing support, psychological support and many other services.
Dear Community Partners and Referral Agents,
Familias en Acción has recently received more than our usual number of referrals for patient navigation from our community sources. In order to best serve our current clients, we are not accepting new referrals requesting patient navigation services until November 15, 2016, unless you are part of the Latina Initiative or needing breast health services.
We will continue to accept calls to answer questions about resources and to connect people with insurance and social services. This short term hold on referrals only applies to clients needing full patient navigation services that are not connected to the Latina Initiative or breast health services.
Thank you so much for understanding our need to provide high quality services to our current clients. We look forward to accepting new referrals in November.
Familias en Acción offers a monthly Spanish speaking support group for survivors at any stage of cancer. This group offers camaraderie, compassion and encouragement among participants, and it is based on a family centered support service. Each month participants can share and learn about different pressing issues in their lives and participate in peer based support and enjoy of activities taught by health experts like yoga, meditation, and nutrition.
