Patient Navigator Program
Familias en Acción helps individuals with and without health insurance, who have been diagnosed with any of the following conditions: diabetes, hypertension, kidney failure, cancer, HIV, substance dependence, any chronic disease, and others. Patient navigators help patients and their families through education about cancer and other chronic conditions, to understand their condition and know the different treatment options that are accessible for them. Patient navigators can assist patients and their families to find resources such as accessing financial help, housing support, psychological support and many other services.
Familias en Acción offers a monthly Spanish speaking support group for survivors at any stage of cancer. This group offers camaraderie, compassion and encouragement among participants, and it is based on a family centered support service. Each month participants can share and learn about different pressing issues in their lives and participate in peer based support and enjoy of activities taught by health experts like yoga, meditation, and nutrition.

Tomando Control de Su Salud
This Living a Healthy Life with Chronic Conditions Self Management Program is a FREE workshop for people with ongoing health conditions such as diabetes, asthma, emphysema, heart conditions, high blood pressure, MS, etc. Participants meet 2.5 hours per week for 6 weeks. It is taught in Spanish and educates and encourages Latinos to manage their chronic illnesses through different self-management tools.
Seed to Supper
This three-week beginning gardening course gives novice, adult gardeners the tools and confidence they need to successfully grow a portion of their own food on a limited budget. Over the course of 3 weeks (2hr classes) participants learn about building healthy soil; planning, planting, caring for their garden and harvesting.
Walk with Ease
This walking program encourages people with and without arthritis to start and maintain an active life year round. During the 6-week program, participants meet once a week in groups of up to 15. Each class begins with a session on health education (arthritis- or exercise-related topics) followed by stretching activities and a 10–35 minute walk.
