What is a Community Health Worker?

CHWs are frontline Public Health Workers who are trusted members of an/or have an unusually close understanding of the community they serve. This trusting relationship enables CHWs to serve as a liaison/link/intermediary between health/social services and the community. Allowing CHWs to facilitate access to services more culturally appropriately, and improve the quality and health service delivery.

CHWs also build individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support, and advocacy. (American Public Health Association, 2008)

Familias en Acción provides health navigation/community health worker services and support to Latino Families that have chronic illnesses. We are currently accepting a limited number of referrals for this program in Washington, Multnomah, and Clackamas.

Who should you refer to Familias en Acción?

Please refer Latino clients with chronic illnesses who need help to manage:

Diabetes

Asthma

High Blood pressure

Clients who have trouble using medications properly

Clients who need help accessing social services

Clients who live in Multnomah, Clackamas or Washington Counties
